Park Ridge Health announces free diabetes program for patients

From the press release from Park Ridge Health:

Hendersonville, N.C. (June 7, 2012) – The Wingate University School of Pharmacy has partnered with Park Ridge Health to offer a special diabetes education program. Upon referral from his or her physician, a diabetes patient can receive one-on-one coaching and medication management assistance with a pharmacist – free of charge – at Park Ridge Health’s Laurel Park facility.

“What this is designed to do is supplement a primary care physician’s guidance,” said Susan Hoy, Director of Pharmacy at Park Ridge Health. “This is additional one-on-one time with a diabetes patient to educate them and allow time for questions and answers.”
The education is provided by pharmacists specially trained in diabetes management at Wingate University. Services available through the free program include blood glucose evaluation, lifestyle coaching, cholesterol management and assistance with smoking cessation. The patient’s progress is communicated with his or her physician after each session.

According to the American Diabetes Association, 25.8 million children and adults in the United States – or 8.3 percent of the total population – have diabetes. An additional 79 million people have pre-diabetes, which means they are at high risk of developing type 2 diabetes.
To find out more about this program or schedule an appointment, patients can call 828.697.4336, ext. 6741, on Monday, Wednesday or Friday.
